Returner Application

These are the instructions for returners. However if you have not participated in the program during the past calendar year (three terms), please fill out the new associate application. If you are new, please return to our new applicant page.

For a complete list of the necessary steps, please download our checklist. We ONLY accept online applications -no paper copies. Therefore please submit all of the following documents as PDF files titled: “LastName_FirstName_FormName.pdf”

  1. Fill out the Returner Application.
  2. If it has been over 12 months since your last PPD, or will be during the term, complete the Annual Health Assessment Form.

  3. Please fill out the Shift Selection Form.
    • You will be given preference over new applicants for the shifts. We will do the best we can to accommodate everyone.
